Tracking and Status Forms
Please DO NOT use old forms. Current versions of all Graduate School forms are available as individual PDF files through the links below. Most are interactive so that you can fill them in and print them from the pdf. (Get Help with PDF)
These forms are used to inform the Graduate School of your progress through your degree program. They also serve as a kind of work contract between you and your advisor/committee, confirming that you’re on the right track. They should be filed in a timely fashion per instructions on each form and/or the timeline to your degree found in the degree requirements. To determine when the tracking forms need to be submitted, please view the Graduate School Tracking Forms Submission Schedule.
Before you arrive
Please be sure your graduate school file contains an official final transcript from your previous college and your signed, witnessed “Patent, Research, and Proprietary Rights Agreement,” which is available on the Web here.
If your final transcripts do not include degree granted, then an official proof-of-degree is also required. Transcripts are not considered official unless they are sent directly to the Graduate School by the degree-granting institution or are received in a stamped, sealed envelope issued by the institution. Michigan Tech graduates do not have to request or pay for their transcripts—the Graduate School has access to them.

- Off Campus Graduate Degree Program Liability Statement—this form is only for students who are earning a Michigan Tech degree by completing their studies off campus through a special agreement between Michigan Tech and another institution.

Forms Related to Theses, Reports and Dissertations
- *NEW* TD-Publishing—This form is now required from all students submitting a thesis or dissertation. It must be submitted two weeks prior to the oral defense and replaces the M7/D9 for those students who wish to publish an ETD

- *NEW* TD-Bindery - complete this form for all requests to publish a thesis, report or dissertation. This is optional for a report. This new form combines the previous Heckman Bindery and invoice form and will automatically perform any calculations for you. Contact Dr. Charlesworth if you are having any technical difficulties.
- *NEW* TD-Review will be returned to graduate students after the Graduate School reviews the thesis or dissertation draft. It is provided here for informational purposes.
- UMI dissertation publishing agreement form located in the UMI document for preparing and publishing your dissertation through ProQuest. Pages 3 and 4 must be completed and submitted to the Graduate School. It is recommended to select publishing option TR-1. Select option TR-2 if publication is to be delayed (an embargo). Prior written approval of the Dean of the Graduate School is required for TR-2 option.
