Graduate Academic Dismissal Appeal Form

Students who would like to be considered for readmission after being dismissed from their graduate program must fill out the form below and submit on-line by pressing the "submit" button at the bottom of the page. All fields are required unless otherwise noted.

  1. What unusual or extenuating circumstances prevented you from being successful?
  2. Describe how these circumstances will change in the future.
  3. What is your plan of action for success?
  4. Do you have missing grades? If so, please describe your plan to address these. (This response is not required if you do not have missing grades.)
  5. (optional) Is there any additional information or documentation that you would like the dean to consider?

Your appeal will be acted on within seven days after it is received. You will be notified of the dean of the Graduate School's decision by email. If the evaluation of your appeal requires that a Graduate School representative speak with you to gain further information, you will be contacted and asked to schedule a personal interview. If your appeal is denied you must sit out for at least one semester, plus a summer if this is your first suspension. If this is your second suspension you must sit out two semesters, plus a summer. You are not eligible to seek reinstatement after a final dismissal.
